Tailored to seniors with complex health needs, ConcertoPACE uses a combination of medical, emotional, and support services enhanced by personalized care.

Our integrated approach helps keep participants safe, comfortable, and healthy. While visiting the ConcertoPACE of Los Angeles center, participants have access to medical services, social interaction, and engagement.

Our team provides or coordinates a range of care for our participants, including but not limited to:

  • All primary and specialty physician services
  • Adult day health center services
  • Family caregiver support
  • Medical equipment
  • Nutritional counseling
  • Medications
  • Recreational therapy
  • Regular assessment of participant needs
  • Rehabilitation services
  • Services in the home
  • Social services
  • Hearing, dental, vision, and podiatry services
  • Transportation to and from the day center and specialty appointments
  • Inpatient and outpatient services

A full team focused on you

ConcertoPACE Interdisciplinary Team (IDT) is made up of multiple clinical and service professionals who meet with you to ensure that every level of care is properly provided and custom tailored to your needs.

Please call 866-383-0688 or fill out our online form. We will set up an appointment to come to your home and fully explain the program and answer any questions.

Enrollment criteria

  • 55 years of age or older
  • Deemed medically eligible
  • Live within our service area
  • Live safely in your community

Cost to you

Once an individual enrolls in ConcertoPACE, all services including medical care are provided by the program. All services that participants receive must be approved by the PACE Interdisciplinary Team. PACE participants may be financially responsible for care and services not authorized by the PACE program.

If at any time you are permanently placed in a nursing home, you may be responsible for a portion of the cost.

Our services are developed and provided to each participant based on their needs. If you feel that you are not pleased, please let us know immediately so that we can take the necessary steps to address your concerns.

If you are not happy or satisfied with any of the services offered through ConcertoPACE, you can let any staff person know.

Filing an appeal

If ConcertoPACE decides not to provide or pay for a service you feel you need, you may ask us to change our decision. This is called an appeal. If you wish to file an appeal we are available to assist you.

By chosing to enroll in ConcertoPACE participants must receive all needed healthcare services. including primary care and specialist physician services (other than emergency services) from ConcertoPACE or a person or company pre-authorized by ConcertoPACE of Los Angeles. ConcertoPACE guarantees access to services, but not to a specific provider.

ConcertoPACE participants may be fully and personally responsible for the costs of any out-of-network services.
